What to take when submitting application (Form D-44)
- Properly completed Form D-44 signed by parents/guardians
- $33 dollars for the fee (cash, check, and certain cards)
- Your social security number
- Your original or copy of your birth certificate, or another valid document
- Valid proof of residency documents
- Your certificate of completion of driver's education (if under 17 and 1/2)
What to take when taking the driving test
Important: Make sure that the vehicle that you'll be taking the driving test in meets the DMV requirements
- Signed permit
- Proof of insurance for the vehicle that you will be taking the driving test in
- A vehicle to take the test in
