A quick checklist on what to take to the DMV

What to take when submitting application (Form D-44)

  • ​Properly completed Form D-44 signed by parents/guardians
  • $33 dollars for the fee (cash, check, and certain cards)
  • Your social security number
  • Your original or copy of your birth certificate, or another valid document 
  • Valid proof of residency documents
  • Your certificate of completion of driver's education (if under 17 and 1/2)

What to take when taking the driving test

Important: Make sure that the vehicle that you'll be taking the driving test in meets the DMV requirements
  • Signed permit
  • Proof of insurance for the vehicle that you will be taking the driving test in
  • A vehicle to take the test in
